Our primary care network (PCN) is committed to providing high-quality, patient-centred healthcare services to individuals and families in our community. As a team, we work together to deliver comprehensive care that covers both physical and emotional health. Our mission is to help individuals in our community achieve better health outcomes and improve overall quality of life through encouraging taking an active role in their healthcare.

The PCN board consists of representatives from each of our locality GP surgeries. We have the practice manager and GP representative from Park View and the practice director and GP representative from Ash Trees. We work collaboratively to ensure what work the PCN does is beneficial for our local population.

Mental Health Team

We are very pleased to be able to have a mental health team that works within both practices. Our mental health team can provide routine and non urgent mental health assessments, advice and signposting, review of mental health medication and support.

We have a First Contact Mental Health Practitioner, a Children & Young Persons Mental Health Practitioner and an APP (Associate Psychological Practitioner).

First Contact Practitioner Team

“First Contact Practitioner – we are Physiotherapists by background and our role is to assess any musculoskeletal (MSK) problems in the over 12s and adults in the Primary Care setting, freeing up the GPs and getting the patient to see the right person at the right time earlier in the process. We do have extended roles and are able to arrange any necessary investigations and/or onward referrals to specialists accordingly. We have mentor support and can arrange appointments with a GP if the problem is not an MSK issue”.

The roles of the First Contact Practitioner Team are not to provide “Physiotherapy treatment” however they will provide a one-off assessment as “First Contact Practitioner”. If Physiotherapy is required, they would then refer on for further input.

PCN Clinical Pharmacy Team

Our PCN Clinical Pharmacy team work alongside a multi-disciplinary diverse team of health care professionals within general practice, providing support for prescription and medication queries.

They focus on providing comprehensive medication reviews, especially for patients with chronic conditions like asthma and heart disease, and those in care homes. The team also works to ensure the safe use of medications, prevent errors, and promote environmentally friendly practices.

Wellbeing Team

Your GP isn’t the only person who can help you feel better – you could improve your health and wellbeing by meeting with a Social Prescriber, Care Co-ordinator or Health & Wellbeing Coach.

Our team know taking care of your health involves more than just medicine. That’s where the Wellbeing Hub comes in. It starts with a conversation, this might be with a doctor or with another person in the practice team who can refer you to the Wellbeing Team.

We will contact you and spend time with you exploring what activities and/or local support could improve your health and wellbeing. You can consider the benefits, and we will assist and support you to plan your next steps.

Paramedic

Our Paramedic provides care to our housebound population with direction from our GPs.

General Practice Assistant – GPA

As part of the wider team in general practice, our General Practice Assistants provide a role supporting all staff within the team and carry out administrative tasks.
